What should I do if my Outcome form contains incorrect information?

If your outcome form includes errors, such as incorrect dates, outcomes, or training details, you should take the following steps:

 

  1. Review the form carefully
     Identify the specific section(s) that contain incorrect information. Make a note of what needs to be corrected.

 

  1. Submit a ticket via the PGMDE Support Portal
    Contact the Healthcare Education Team (HET) by submitting a ticket through the PGMDE Support Portal.

    In your ticket, clearly outline:
    • The nature of the error
    • The correct information
    • Any supporting details or documentation that may help verify the correction

 

Act promptly, timely reporting helps ensure your training record is accurate and avoids delays in progression or certification.
