I am Nuclear Medicine trainee but not London trainee, where do I get a Form R from for my ARCP?

All Nuclear Medicine ARCPs are done via the same ARCP panel. Staff from the London team of NHS England will send out correspondence to you to advise you of the date 6 weeks prior to allow you enough time to ensure your ePortfolio is up to date.

You will be responsible for making sure all your information can easily be found on your ePortfolio. You might want to consider creating a folder clearly marked ACRP with the relevant year.

You should have received a link for Tis Self Service to create a current Form R.  For the purposes of revalidation at the time of ARCPs, you need up update your Form R.  This should be done no earlier than 4 weeks prior to your ARCP date.  After you complete the the Form R, you then need to upload it to your ePortfolio so the panel can see this on the day of your ACRP.
