I am a trainee who has recently changed my Designated Body/ Responsible Officer. What should I be providing them for revalidation purposes.

The standard vehicle for transferring fitness-to-practise information about a trainee doctor is the doctor’s most recent ARCP outcome form. 


This form includes a comment on whether or not there were any known unresolved concerns about the doctor’s fitness to practise at the time of the review.


Trainees are expected to maintain their own records of ARCP outcome forms in either digital or paper format and to provide these directly to their new Designated Body/ RO. 


This is set out in the nationally agreed COPMeD process for TOI 


We are unable to provide ARCP outcome forms or complete individual RO-to-RO TOIs unless there are exceptional circumstances.
