Submitting your Form R (Part A and Part B) for ARCP

Modified on Thu, 17 Sep at 2:38 PM

What this article is about
This article explains why Form R is required and what you need to do before ARCP.

Who this is for
Resident doctors in training who are required to submit Form R as part of ARCP, revalidation or training progression processes.

What you need to know

  • Form R is used to support ARCP, revalidation and training record processes.
  • You must submit the required Form R sections by the deadline set by your school or programme team.
  • The form should reflect your full scope of practice since your last review.
  • You must declare relevant information honestly, including any significant events, complaints, investigations, health or conduct matters where required.
  • Failure to submit Form R may result in the panel being unable to make a progression decision.
  • If Form R is incomplete or missing, an Outcome 5 may be issued while the missing information is requested.
  • Some local processes may combine Form R with a trainee self-declaration form.

What to do

  1. Check which Form R sections you need to complete.
  2. Complete all mandatory fields carefully.
  3. Include your full scope of practice since your last ARCP or review.
  4. Make any required declarations honestly and clearly.
  5. Upload or submit the form using the route specified in your ARCP guidance.
  6. Keep a copy of the submitted form.
  7. Tell the ARCP team promptly if you cannot access the form or believe you have made an error.

If this does not work
If you cannot submit Form R, raise a query before the deadline. If you have submitted the wrong form or missed information, tell the ARCP team as soon as possible.

Who to contact
Use the PGMDE Support Portal for Form R submission queries. Contact your educational supervisor or Training Programme Director if you are unsure what to declare or how the information may relate to your training.

What HET can and cannot help with
HET can help with

  • Confirming the submission route and deadline.
  • Checking whether a Form R has been received where HET holds the record.
  • Advising whether missing Form R information may affect ARCP administration.
  • Directing revalidation-related queries to the correct route.

HET cannot help with

  • Completing Form R on your behalf.
  • Advising you to withhold information that should be declared.
  • Providing legal, regulatory or employment advice.
  • Removing relevant declarations from required ARCP or revalidation processes.
