How are LTFT resident doctors funded?
All LTFT slot share payments are paid to the Trust quarterly via the Finance and Activity Payment (F&AP) schedule. Payments are made pro-rata across the year and not in one lump sum. Payments will be adjusted each quarter and a detailed breakdown on how LTFT payments are made to the Trust are confirmed in the finance pack that is sent to the Trust each quarter.

Reduced sessions in a Full-Time Post
- If the resident doctor is working reduced sessions in a full-time post, then there is no funding due from the LTFT budget to the Trust.
- This is because the training post that the LTFT resident doctor occupies is already fully funded from Tariff funding.
- The cost to the trust will therefore be nil as the funding for a full-time resident doctor is already in place but the resident doctor is working less than full time.
Slot share funding
LTFT slot-share funding is comprised of two elements, independent of the funding type of the post:
- Salary support funding for the WTE above 1.0
- Placement fee top-up, again based on the WTE above 1.0
Salary Support funding
Salary support funding is an additional contribution from NHSE towards the salary costs of the trainees in the slot-share, being additional to the salary support portion of any post funding. This is only to be used as a contribution toward salary costs. Given that, by definition, a slot-share will include two LTFT in-post trainees, the salaries to be supported are those of these two trainees.
Placement fee top-up
The placement fee top-up is additional to any placement fee provided as part of post funding. Posts that are Trust funded or funded non-tariff, will have no placement fee as part of post funding; yet there will still be a placement fee top-up as part of slot-share funding.
The placement fee is to cover funding for direct costs involved in delivering education and training by the placement provider, for example:
- direct staff teaching time within a clinical placement
- teaching and trainee facilities, including access to library services
- administration costs
- infrastructure costs
- educational supervisors and assessors
- pastoral and supervisory support
- time for clinical exams
- health and well-being (excluding any occupational health assessments that are carried out by the employer and funded separately)
- formal examination
- staff training and development relating to their educational role
The placement fee does not cover:
- tuition costs
- items funded under education support such as:
- foundation programme directors
- foundation programme administration support staff
- heads of schools
- programme directors
- relocation costs and exceptional travel costs
- study leave costs
- Slot sharing works on the basis that two resident doctors share one full-time post and manage any out-of-hours between them. Each doctor may work up to 80% whole time equivalent (WTE) and is paid as an individual resident doctor on the basis of actual contractual hours.
- Slot share – the first 1 WTE is funded through post funding (50% salary support plus placement fee). London will fund anything above 1 WTE for slot shares.
- Both trainees can work between 0.5, 0.6, 0.7 and 0.8. London will not fund any placements below or above these amounts.
Slot-share example for tariff funded post:
Trainee A – works WTE of: | 0.7 |
Trainee B – works WTE of: | 0.6 |
Total WTE worked by Trainees (0.7 + 0.6): | 1.3 |
Trainee A funded by post funding at | 1.0 |
Trainee B funded through LTFT budget at | 0.3 |
Total funding to Trust for slot share (1.0 + 0.3) | 1.3 |
Please note that NHS England does not provide incentive payments to NHS Trusts for taking on LTFT trainees.
Maternity Leave payments
London Trusts no longer receive £12,000 for each existing LTFT trainee that takes maternity leave.
How are academic LTFT trainees funded?
Academic LTFT resident doctors are funded via the academic funding schedule and will not appear on the LTFT funding report. If academic LTFT trainees are slot sharing with a non-academic LTFT resident doctor the non-academic LTFT trainee will be funded as reduced sessions.
On-calls, sick leave and paternity leave
The Trust is responsible for paying on-calls, sick leave and maternity leave.
