Trainees who are ready to CCT will need to contact the Royal College of Ophthalmologists https://www.rcophth.ac.uk/ for the CCT form, fill in the required fields, then submit the form to the PGMDE Support Portal to be signed off by the Head of School. The Ophthalmology team will then return to the trainee to send to the College.
