Form R is a mandatory document for trainees, required as part of the Annual Review of Competence Progression (ARCP). It consists of two sections:
- Part A: Records your personal and training details.
- Part B: Includes declarations regarding health, probity, and any significant events or concerns during your training.
TSS (TIS Self-Service):
Public Health Pan-London has now migrated to the TIS Self-Service (TSS) platform. Trainees are expected to complete both Part A and Part B of their Form R through TSS.
To access the platform, please log in at: https://trainee.tis.nhs.uk/
Once completed:
- Download a copy of your Form R from TSS.
- Upload the downloaded copy to your ePortfolio to ensure it is available for your ARCP review.
If you need your Form R to be reset, please contact the Healthcare Education Team (HET) by submitting a ticket via the PGMDE Support Portal.

Previous versions of Form Rs will not be accepted at ARCP.
